Death Certificates

The Medical Certificate of Death, Form 16, is a form that a coroner or physician or registered nurse practitioner can fill out. This form is given to the funeral home and is registered with the Province of Ontario along with a Statement of Death, form 15, that you will fill out and sign with your funeral director. These forms are sent to the Registrar General in Thunder Bay, Ontario. The funeral home is not permitted to give you a copy of these forms. Your service provider will, however, give you a Proof of Death Certificate. These are recognized by all agencies as a proof of the death, i.e., bank, insurance companies, lawyers, ministry of transportation, etc.
There are few circumstances, however they do exist, where an agency may require the Medical Certificate of Death. A formal request to the province is required. Your service provider can aid you in retaining these papers.
